At the Hazel Burns Hospice, Case Management begins with a Case Manager assessing the Client’s physical, mental, emotional and spiritual condition. Case Management is provided to each Palliative Client by a Registered Social Worker.

Case Management includes:

• Organization of Care Team Planning
• Advance Care Planning
• On-going monitoring of Client’s physical, mental, emotional and spiritual condition
• Community liason
