At Hazel Burns Hospice, a Case Manager will receive all client referrals, and contact either clients or designated care giver contacts to set up a home visit. At the home visit. clients will receive an Information Package containing descriptions of current programs and related services, as well as items of interest for those living with life limiting conditions, their families and their care providers.
Our Case Management program offers:
Referrals to Hazel Burns Hospice program and services;
Assistance with Advance Care Planning;
- On-going monitoring of Client physical, mental, emotional and spiritual condition;
- Referrals to service providers, organizations and agencies that support needs in the areas of hospice service above;
- Liaison with other providers in the Integrated Community Palliative Care Team networks;
- Links to education, training and research.
